How do I get every hidden Esper?
Twelve Espers are basically handed to you as part of the plot.
Golem: Buy it from Jidoor's Auction House (available in the WoB) for 20,000 Gil after you get the airship. The wares are random so keep returning. You can still get it in the WoR if you missed it in the WoB.
Zona Seeker: Buy it from Jidoor's Auction House (available in the WoB) for 10,000 Gil after you get the airship. Again, the wares are random, and you can still get it in the WoR if you missed it in the WoB.
Seraph: After getting the Airship in the WoB, go to Tzen, and a man hiding in the trees in the northern part of town will offer to sell it to you for 3000 Gil. In the WoR, he will offer it for only 10 Gil.
Quetzalli: After getting the airship in the WoR, go to Solitary Island, where Celes began the WoR, and head to the beach south of the hut.
Fenrir: You receive this automatically while recruiting Terra. See [21]
Valigarmanda: You receive this automatically while recruiting Umaro. See [21]
Midgardsormr: You receive this automatically while recruiting Umaro. See [21]
Lakshmi: You receive this automatically after recruiting Relm. See [21]
Alexander: Form a party of four that includes Cyan, and sleep in the room with beds in Doma Castle. The rest of your party must go through Cyan's dreams. Complete this sequence and Alexander will appear in the throne room of Doma.
Phoenix: You receive this automatically after recruiting Locke. See [21].
Ragnarok: Head to Narshe with Locke in your party. In one of the houses there, a man will offer you a choice between the Ragnarok Esper and the Ragnarok sword. See [28] for information on this choice.
Odin: After getting the airship in the WoR, travel underground using Figaro Castle. When the navigator comments on the unusual terrain, stop and exit through the open cell in the prison. Explore this area to find the Ancient Castle; Odin is inside.
Raiden: After getting Odin, walk four steps down from the right-hand throne and press A. This will open a secret passage. Descend and examine the statue of the queen; she will "upgrade" Odin into Raiden. Note that you will lose Odin when you do this.
Bahamut: A monster called Deathgaze is hiding over two random spots in the WoR. If you manage to fly the airship over either of these spot, you will fight it. Deathgaze tends to run away, but its HP does not replenish between battles, so just keep chasing it down. Defeat it and to get Bahamut.
A good way to find Deathgaze is to have the airship face due north or south. Press left or right in the D-Pad very slightly, so the airship is facing just slightly off to the side. Now, hold A. You will fly over every tile on the map, which means you will eventually find it.
Crusader: You receive this after defeating the 8 Elemental Dragons. [45] gives you their locations.
Leviathan: Take the ship from Nikeah and South Figaro after getting the Airship. You will get into a battle with Leviathan. See [46] for battle strategy.
Gilgamesh: Buy the Excalipoor in the Jidoor Auction House for 500000 Gil. Make sure you talk to the person outside the Auction House first, or the sword will not appear. After that, bet the Excalipoor in the Coliseum to fight Gilgamesh.
Cactuar: Defeat 10 Cactuars in the desert south of Maranda. After that, step on a certain tile in the southwest of the desert to trigger a battle with Gigantuar. See [47] for battle strategy.
Diabolos: Reach the end of the Dragon’s Den to receive Diabolos.

Are any Espers permanently missable?
The 3 permanently missable Espers are Ifrit, Ragnarok, Bahamut. While it is theoretically possible to miss Ifrit, you would have to be blind not to take it in the Magitek Research Facility.
If you choose the Ragnarok sword over the Esper, then you have missed the Esper forever. See [28] for details on this choice.
You get the Bahamut Esper after defeating Deathgaze on the world map. However, if you defeat Deathgaze in the Soul Shrine first, then the Deathgaze on the world map will not appear, which means you have lost Bahamut forever. So make sure you defeat Deathgaze before entering the Soul Shrine!
